2 – Hauling Permit (Brand Inspection or Brand Inspection Certificate)
3 – Health Certificate must include VS statement for previous 5 days
4 – Health Certificate must include VS statement for previous 30 days

– Health Certificate must have attached a list of all vaccinations, date
administered &
type of vaccine used

– Brand Inspection
Certificate required; call 406-444-2045 to obtain one if your originating state
does not issue same

– Along with Negative Coggins (EIA), a negative test certificate for CEM
(Contagious Equine
Metritis) is required
8. Annual Equine Import Permit required; must have
lifetime Brand Inspection Certificate. Go to

9. Pre-entry Permit required. Call the Permits number listed.
10. CVI must be accompanied by Identification and temperature of the horse
